02.01.011 - Legal Name Requirement for Official Student Records

Policy Number: 02.01.011
Policy Title: Legal Name Requirement for Official Student Records
Policy Status: Active

Policy:

Axon Education maintains accurate and permanent student records. The institution requires that all official academic and institutional records reflect the student’s legal first and last name as documented on government-issued identification or official court documentation.

The student’s legal name will be used on all official records maintained or issued by Axon Education, including but not limited to transcripts, enrollment agreements, certificates of completion, graduation records, student academic files, financial records, and any documentation issued for purposes of certification, licensure, employment verification, or background screening.

Students are responsible for providing their correct legal name at the time of enrollment. If a student’s legal name changes after enrollment, the student must submit official legal documentation to request an update to institutional records. Acceptable documentation may include a court order, marriage certificate, or other legally recognized documentation reflecting the name change.

At the discretion of the institution, a preferred name may be used for limited internal or non-official purposes. The use of a preferred name does not replace the student’s legal name on official academic or institutional records.

This policy ensures that student identity and credentials can be accurately verified by external stakeholders, including employers, licensing boards, certification agencies, and background screening entities.

Meta

This policy supports compliance with DEAC Accreditation Standards, including Standard XV (Facilities and Records Maintenance), applicable State of Texas legal name requirements, and other applicable national and state regulatory requirements related to the maintenance and verification of student records.
